Compensation Range: $46,000 to $53,000
Location: Brooklyn, Bronx, Queens, and Staten Island
We are representing multiple case management positions within multiple boroughs and different populations.
Candidates will:
- Conduct detailed intake, initial-in-home assessment and reassessment of clients, including closing cases.
- Develop individualized care plans based on client needs and preferences in collaboration with client's family member(s).
- Assess overall wellbeing of clients including social, emotional, daily living skills and financial needs in- person and over the phone.
- Establish service linkages, make necessary referrals based on un-met needs, and monitor service delivery in order to successfully implement individualized care plans.
- Maintain accurate client records including detailed case notes, assessments and other data.
- Perform daily client intakes.
- Provide benefits and entitlements counseling, assist with applications and enrollments, and follow-up to ensure client is receiving benefits and entitlement.
Qualifications:
- BA, BS, BSW required
- Bilingual preferred.
- Must be proficient in Microsoft Word, Excel, and Outlook.
- Excellent organizational skills.
- Experience working with older adults preferred.
