Compensation Range: $46,000 to $53,000

Location: Brooklyn, Bronx, Queens, and Staten Island

We are representing multiple case management positions within multiple boroughs and different populations.

Candidates will:

  • Conduct detailed intake, initial-in-home assessment and reassessment of clients, including closing cases.
  • Develop individualized care plans based on client needs and preferences in collaboration with client's family member(s).
  • Assess overall wellbeing of clients including social, emotional, daily living skills and financial needs in- person and over the phone.
  • Establish service linkages, make necessary referrals based on un-met needs, and monitor service delivery in order to successfully implement individualized care plans.
  • Maintain accurate client records including detailed case notes, assessments and other data.
  • Perform daily client intakes.
  • Provide benefits and entitlements counseling, assist with applications and enrollments, and follow-up to ensure client is receiving benefits and entitlement.

Qualifications:

  • BA, BS, BSW required
  • Bilingual preferred.
  • Must be proficient in Microsoft Word, Excel, and Outlook.
  • Excellent organizational skills.
  • Experience working with older adults preferred.
