Schedule: Monday to Friday (Remote)

Atlantic Group is hiring a Behavioral Health Care Coordinator (Remote) for our client. In this fully remote role, you will engage members by phone, conduct behavioral health screenings, coordinate referrals, support post-discharge follow-ups, and help members access needed services and community resources. Ideal for professionals with strong communication skills and experience in behavioral health or care coordination.

Responsibilities as the Behavioral Health Care Coordinator (Remote):

  • Member Engagement & Education: Conduct telephonic outreach using motivational interviewing, educate members on conditions, treatment options, and how to navigate behavioral health resources.
  • Care Coordination: Manage referrals, link members to behavioral health and substance use services, and advocate for timely access to appropriate care.
  • Screenings & Follow-Up: Perform telephonic screenings, wellness outreach, appointment reminders, and post-discharge transitions-of-care calls.
  • Triage & Communication: Receive and route referrals, handle transferred calls from internal teams, and collaborate with providers, programs, and community agencies.
  • Documentation & Operational Support: Maintain accurate documentation in systems such as Jiva, support workflows and SOPs, and assist with administrative tasks and process improvements.

Qualifications for the Behavioral Health Care Coordinator (Remote):

  • Education: Associate’s degree in a healthcare-related field required (Bachelor’s degree preferred).
  • Experience: Prior experience in behavioral health coordination, care management, call centers, healthcare administration, or customer service, with Medicaid population experience preferred.
  • Technical Skills: Proficiency in Microsoft Office (Teams, Outlook, Word, Excel, PowerPoint) with ability to document effectively in care management platforms.
  • Industry Knowledge: Familiarity with behavioral health and substance use disorder treatment, community resources, and medical/behavioral health terminology preferred.
  • Skills & Attributes: Strong motivational interviewing and communication skills, detail-oriented, and able to prioritize effectively while working both independently and collaboratively.
