Health and Recovery Plan Supervisor (HARP)/HH+ Supervisor Role

Title of Position: Supervisor Role: Health Home Program

Compensation Range: $60,000

Location: New York, NY

We are representing an organization that assists with Medicaid patients.

Candidates will:

  • Monitor and complete reports on care coordinator performance, productivity, and quality.
  • Train onboarding care coordinators.
  • Conduct supervisions with care coordinators at minimum of monthly basis.
  • Promote self-care for care coordinators.
  • Develop new trainings and tools for care coordinators to improve quality and efficiency.
  • Hire, train, and supervise Care Coordinators who provide treatment education.
  • Collaborate with other members of the project team in development, implementation, and ongoing review of policies, procedures, and standards of care.
  • Review cases with staff to ensure accurate assessment of client needs and appropriateness of intervention plan.
  • Ensured staff deliver Evidence Based Models.


  • Licensed professionals such as RN, LCSW, Licensed Mental Health Counselor (LMHC), License Psychoanalyst, or License Creative Art Therapist.
  • 2-years’ experience in the healthcare, community health, social work, supportive housing, or other social service interventions.
  • Excellent customer service, organizational, interpersonal and communication skills.
  • Able to multi-task and work under multiple priorities and deadlines in a fast-paced environment.
