Responsibilities

 

  • Identify client care needs by establishing comprehensive assessment of the health needs of children in care through direct means of interaction.
  • Develop a comprehensive coordinated care plan based on that assessment in conjunction with the medical home staff and treatment team.
  • Establish for children in care a medical home that will address current and future health care needs.
  • Act as a liaison between multiple providers in the provision of health services for our children in care.
  • Establish a compassionate environment by providing emotional, psychological, and spiritual support to clients and families.
  • Obtain any necessary consent.
  • Promote client and family’s independence by establishing goals, teaching clients, families and caretakers to understand condition, medications and self-care skills.
  • Resolve client problems and needs by utilizing multidisciplinary team strategies.
  • Document client care services by charting in patient and department records; on-going auditing of medical records to ensure compliance with regulatory agencies.
  • Communicate with clients and multidisciplinary team by providing client related health information.
  • Support caregivers as necessary in following the AAP periodicity schedule for routine care.
  • Compile child’s health information for use in court hearings.
  • Ensure compliance with regulatory agencies’ rules for children in care who are on psychotropic or any other medications.
  • Assist and support child in receiving special services such as early intervention and waiver services.
  • Maintain a cooperative relationship among health care team by communicating information, responding to requests, building rapport, participating in team problem-solving methods.

 

Requirements

 

  • Graduate of an accredited nursing program and with a license in good standing.
  • Must demonstrate firm command of nursing principles and practices.
  • Pediatric and mental health experience preferred.
  • Strong communication and advocacy skills.
  • Must have a current American Heart/ or American Red Cross CPR Certificate to deal with infants, adolescents and adults.
  • Must be able to accompany youngsters to various appointments.
