Nurse Case Manager

Responsibilities

 

  • Take all-inclusive Intake Assessment, including preliminary medical history and ongoing medical apprehensions, prior to client commencing services with the Company
  • Evaluate the clients home health needs, safety and care instructions, legal documents and discuss any of the client family’s questions or concerns
  • Complete accurate and detailed clinical documentation within established time frames
  • Create care plan in conjunction with patient and family to assist in achieving optimal resolution of needs
  • Assume responsibility for the maintenance of patient communication and records from intake until case stability
  • Engage in dialogue with care team members during case onset regarding the caregivers’ ability to follow care plan and possible need for training
  • Establish and maintain professional working relationship with health care providers and other members of the disciplinary teams in providing patient and family care
  • Communicate regularly with members of the clinical team and support fellow team members to ensure the best outcomes for our clients and families

 

Requirements

 

  • NY State RN License, BSN Ideal
  • 3 + years of professional nursing experience, with 2+ years in a multi-faceted health care setting
  • 1+ year of experience in the home care arena
  • Knowledge of the principles and practices of primary nursing and home care
  • Experience with the geriatric population is strongly preferred
  • Strong assessment and clinical skills
  • Technologically savvy, particularly with Microsoft Office suite and other electronic medical record system
