Family Specialist: MEHR

Responsibilities

 

  • Oversee the diverse case management needs of approximately 15 family residents funded through DOHMH/NYNY3.
  • Responsible for providing follow up aftercare services for one year to all NY3 families moving on from MEHR, including phone, email and/or in person contact every three months.
  • Work with family residents to develop and implement individualized service plans and goals based upon the specific service needs of the family resident.
  • Ensure that family residents are meeting the expectations of their contract and support them as they develop life and work skills.
  • Assist family residents in making referrals to appropriate providers for housing, educational and vocational services.
  • Assist in the completion of NYCHA, Mitchell Lama, Housing Connect and other housing applications for all families serviced.
  • Serve as a liaison between families, MEHR, and ACS as necessary.
  • Provide advocacy and referral to entitlements, shelter, legal assistance, medical, mental health and other emergency needs, as well as referrals for on-going follow-up needs.
  • Provide direct service, including individual and group counseling to residents on an as needed basis.
  • Facilitate bimonthly Women’s Group and/or Parenting Journey workshops.

 

Requirements

 

  • BA Degree plus two years’ experience working in a residential program with young adults preferred.
  • Understanding of the special needs of families and a sensitivity to ethnic and cultural diversity.
  • Must be able to communicate clearly and advocate on behalf of family residents.
  • Flexible hours to insure access and availability. Some weekends and evenings as necessary.
  • Must have a commitment to work from a strength-based, youth development and trauma-informed perspective.
