Responsibilities

 

  • Construct and execute individual service plans that indicate communally agreed upon short- and long-term goals.
  • Acquire and preserve all government benefits/entitlements and services for each client on caseload.
  • Make suitable referrals, such as medical care, mental health services, vocational services, recreational activities, and substance abuse and transportation services.
  • Perform weekly home and/or office visits and hospital visits
  • Escort clients to medical appointments.
  • Handle medical and personal emergencies of appointed clients and make any modifications needed to the care of the client based on new needs.
  • Carry out reassessments of service plans on a regular basis to guarantee ongoing awareness of the client’s medical conditions and service changes needed.
  • Maintain an accurate, confidential and current chart on all clients on caseload.
  • Prepare a monthly narrative report for assigned caseload on a timely basis.
  • Attend regularly scheduled supervision meetings with the Program Director.
  • Participate in the weekly team meeting and other division meetings.
